The 22-year-old forward is expected to undergo medical in a bid to finalise the move ahead of Friday’s transfer deadline.
The deal also includes a 10 per cent sell on clause.
Forest have been ready to sell Johnson since the end of last season and had knocked back two bids from Brentford for their homegrown star, plus an initial enquiry from Tottenham on Thursday.
