New details about 2023 Titan submersible implosion revealed in Netflix documentary

The implosion of a submersible resulting in the loss of five lives near Newfoundland two years ago was due to “multiple contributing factors,” according to Coast Guard investigators on Tuesday. They emphasized that the tragedy was entirely “preventable.”

The Coast Guard released its findings in a 300-page report — the result of a two-year investigation.

The planned dive on June 18, 2023, ended in tragedy after the Titan lost communication with its OceanGate handlers, setting off a frantic search.

Days later, it was determined the submersible imploded and instantly killed all five people on board, officials said.

“This marine casualty and the loss of five lives was preventable,” explained Jason Neubauer, the chairman of the U.S. Coast Guard Marine Board of Investigation responsible for examining the Titan disaster.

“The two-year investigation uncovered several contributing factors to this unfortunate event, offering important lessons to avert such occurrences in the future. There is a need for improved oversight and distinct options for operators working on innovative concepts outside the current regulatory framework.”
