Simone Holtznagel has shared her heartbreak after her beloved grandmother died on Thursday at 100.

The 31-year-old model posted a gallery of photos to social media on Saturday which captured her spending quality time with her gran and captioned it with some heartbreaking words.

‘100 beautiful years. Our matriarch, our rock, our heart. After 100 remarkable years, my beautiful Grandma has left us,’ Simone began.

‘We are so lucky. You were pure love, strength, and grace. My heart will always carry yours, all of our hearts will.’ 

Simone added she would miss her beloved grandmother forever.

‘My Grandma shaped our family and her love will carry us forward forever… I miss you so much already, Granny,’ she wrote.

Many of Simone’s celebrity friends took to the comment section to share their condolences.

‘Sending you and your family love and strength in this hard time lovely,’ wrote Married At First Sight star Domenica Calarco and Jasmine Stefanovic shared a loveheart emoji.

‘I’m so sorry honey. Sending you so much love,’ added Real Housewives of Sydney’s Sally Obermeder. 

It comes after Simone scattered the last of her late close friend and mentor Charlotte Dawson’s ashes at the beach, on the 11th anniversary of her death.

In February, Simone shared a photo to Instagram of the scenic beachside where she scattered the last of Charlotte’s remains and captioned it with some heartfelt words. 

‘After 11 years, early this morning I walked to my favourite beach with my baby to finally scatter the last of Charlotte’s ashes I have, after scattering them in meaningful places around the world over the last few years.’ Simone began.

‘The years between then and now have brought moments of clarity, grief, healing… everything in between.’

Simone added that even though her close friend had been gone for over a decade she still missed her every day.

‘Each year I have learned something new about how to carry her memory with me. What I have carried for all this time is the unique bond, one still alive within me even as her ashes are released back into the world.

‘Eleven years feels like a lifetime ago but also like it was yesterday. We think of you every day and I wish you were here.’

Simone also drew half a love heart in the sand and wrote Charlotte’s initials, CD. 

Charlotte was a popular TV presenter and a judge on Australia’s Next Top Model. 

She was found dead in her Sydney Woolloomooloo home on February 22, 2014.
