NTSB Probes Multiple Failures in Fatal LaGuardia Crash: Chief Investigator Reveals Ongoing Investigation

The tragic plane crash at LaGuardia Airport likely involved “multiple failures,” according to the head of the investigation team.

Jennifer Homendy, chair of the National Transportation Safety Board, stated that it is “too early” to solely hold the air traffic controller accountable. The controller was overheard admitting they “messed up” shortly after the Air Canada flight collided with a fire truck, resulting in the deaths of the two pilots and injuries to 41 others.

“In our investigations, we consistently find that no single error leads to such a disastrous outcome,” Homendy shared on Fox News’ “Fox & Friends.”

“Our aviation system is designed to be highly safe, and it typically requires multiple failures for an incident of this magnitude to happen,” she added.

“Therefore, our approach will be thoroughly comprehensive,” she emphasized.

The catastrophe occurred when an Air Canada aircraft crashed into a fire truck. The vehicle had been cleared to check on another plane that had aborted its takeoff.

Just moments before the collision, an air traffic controller can be heard frantically telling the fire truck to stop.

Roughly 20 minutes later, the controller appears to blame himself.

“We were dealing with an emergency earlier,” the controller could be heard saying. “I messed up.”

About 40 passengers and crew members on the regional jet from Montreal, as well as two people from the fire truck, were taken to hospitals.
