CPSC Warns Parents: Throw Away Slide Playsets After Toddler Death

BETHESDA, MD – Federal safety officials are telling parents and caregivers to immediately stop using a children’s slide playset after a child died in an incident tied to what regulators describe as a potentially deadly design hazard.

The U.S. Consumer Product Safety Commission issued the urgent warning Thursday for the Model WPX-4 Slide Playset, manufactured by Wenzhou Yidian Amusement Equipment Co., Ltd. According to the CPSC, the playset contains several openings that are large enough for a child’s body to slip through but too small for the child’s head, creating a serious head and neck entrapment risk.

Regulators said the product has been linked to the death of a 17-month-old child who became trapped in the playset.

What to look for — and what to do

The plastic playsets, intended for use indoors or outdoors, feature multiple activity areas, including a tunnel, slide and basketball hoop. Some versions also have a vehicle-shaped section attached to the side. The sets were sold in several colors, including red, navy, coffee, gray, green and yellow.

Consumers can identify the product by checking for the model number WPX-4 and the manufacturer’s name, which appear on a label attached to the playset.

The CPSC is advising caregivers to take the following steps:

Stop using the playset right away

  • Dispose of it — do not resell, donate or pass it along to another family

  • Manufacturer refuses to cooperate

    The situation is complicated by the fact that Wenzhou Yidian Amusement Equipment Co., Ltd. has not agreed to an acceptable recall of the product. Because of that, the CPSC is not offering a refund or replacement — it is simply urging consumers to dispose of the playset entirely.

    The warning was issued Aug. 13, 2026, and assigned product safety warning number 26-703.
