An incident in Sullivan County, Tennessee, has led to multiple charges, including vehicular homicide and DUI, against a man following a fatal accident on Monday.

According to a report from Tennessee Highway Patrol (THP), a 2013 Chevrolet Equinox was traveling northbound on Highway 421 just before 4 a.m.

According to the Tennessee Highway Patrol, a passenger was partially outside a window as the vehicle neared Shady Ford Road. The car veered off the road into a ditch on the right side.

During the vehicle’s movement through the ditch, the passenger, named by authorities as 21-year-old Fatima Guadalupe Valenzuela-Flores, allegedly fell out of the window.

Valenzuela-Flores reportedly died at the scene.

The driver, identified as 21-year-old Jacob Dylan Chambers, reportedly turned the vehicle around, returned to Highway 421, passed by the accident location, and continued to a boat ramp parking lot where he stopped the car. The site is accessible via the bridge over South Holston Lake.

Chambers was charged with the following, according to THP:

  • Vehicular homicide
  • DUI
  • Driving on a suspended license
  • Reckless endangerment

A second passenger was uninjured in the crash, THP stated.
