'America's best doctor' learns fate after stealing more than $36M

An Arizona physician has been sentenced to seven and a half years in prison after prosecutors said his companies generated $36.5 million through a sweeping healthcare fraud operation in only six months.

Dr. Irfan Fazil, 54, was described as the architect of a multimillion-dollar scheme involving fabricated medical services, so-called “phantom billing” and the transfer of stolen funds to accounts overseas, including in Dubai and Pakistan.

Fazil faced a lengthy indictment filed in June that included felony allegations of fraud, theft, conspiracy and drug-related offenses. He pleaded guilty several months later.

The former kidney specialist, who had twice been named a top doctor in his field, was sentenced last week in Yuma County Superior Court, according to Arizona’s state attorney general.

Arizona Attorney General Kris Mayes said Fazil will also serve seven years of probation once he completes his prison term. The case centered on alleged fraud against the Arizona Health Care Cost Containment System, the state’s Medicaid program.

“Dr. Fazil abused his position as a physician to steal more than $36 million from a taxpayer-funded program that exists to provide health care to Arizona’s most vulnerable residents,” Mayes said.

The details paint a picture of a medical empire that appeared highly successful on the surface. Fazil and his wife enjoyed an extravagant lifestyle while he cultivated an image as a respected healthcare provider.

The couple even hosted their own health-focused YouTube program and produced a polished charity magazine. Their reported assets included private aircraft and luxury vehicles.

The Attorney General’s Office said Fazil used three businesses—Bio Family Clinic Inc., Yuma Mobile X-Ray and Yuma Kidney and Dialysis—to submit tens of thousands of dollars in allegedly false radiology claims to AHCCCS.

Fazil, a two-time “top kidney doctor,” received a seven-and-a-half-year prison sentence followed by seven years of probation and was ordered to surrender all of his assets.

According to Mayes, Fazil used Bio Family Clinic Inc., Yuma Mobile X-Ray and Yuma Kidney and Dialysis to submit radiology claims to AHCCCS from July 1 through December 31.

In less than a year, the businesses received more than $36 million from Arizona’s Medicaid system for X-rays, electrocardiograms, CT scans, ultrasounds and other purported medical services.

The attorney general’s office said the sharp increase followed a period when the doctor’s businesses had collected about $4.7 million. Payments on their claims later jumped by nearly 671 percent, prosecutors said.

Investigators reviewed the records of the 27 patients associated with the highest billing levels. In one especially striking case, a single patient’s file listed hundreds of radiology procedures, including more than 1,700 X-rays over a matter of months.

The attorney general’s office called that total “a physically impossible number” and said such exposure would have posed a serious risk to the patient’s health.

“He billed for thousands of X-rays and other medical treatments that could not possibly have been performed, and treated AHCCCS as his personal bank account,” Mayes said in the statement.

Kara Carson, who said she and her father had been Fazil’s patients for roughly 15 years, recalled the moment she began questioning the records. Her insurance company contacted her to ask whether she was still using a wheelchair and had tubes in her throat.

“As you can clearly see, there is no tube in my throat. I never had one. I am not in a wheelchair. I very much can walk just fine, and I have MS, is what I’ve been told. That was all news to me,” she told AZFamily.

Fazil also wrote his wife multiple prescriptions for drugs including clonazepam while she was outside the US, before picking them up himself from the pharmacy, the office said.

In June, he was dragged from his clinic in handcuffs and booked into Yuma County Detention Center, where a judge set his bond at a jaw dropping $50 million.

He later pleaded guilty to felony charges including fraudulent schemes and artifices, illegal control of an enterprise, theft and illegal procurement of a dangerous drug.

At his October 2 sentencing, Fazil’s punishment went beyond years behind bars, with a judge also ordering him to forfeit all of his assets. 

Among them are real estate, medical facilities, vehicles, multiple aircraft, jewelry, luxury handbags, US currency, cryptocurrency and gold bars, according to the attorney general.

‘My office will continue to aggressively prosecute fraud in government programs to protect tax dollars,’ Mayes said.

He dispensed health advice on everything from probiotics to prostate health for nearly a decade on his own YouTube program, Dr Fazil’s Show. 

The couple even published their own health magazine, further burnishing Fazil’s medical credentials while showcasing the family’s charitable work in the Yuma community. 

But allegations emerging amid a major Medicaid fraud crackdown in June 2026 revealed a jarring contrast to the trusted reputation the top doctor had spent 15 years building in America. 

Following Fazil’s indictment, former patients and employees told Our News Outlet they felt furious and betrayed, accusing him of exploiting their trust for profit.

Kamay Bowman, 52, a patient for four years, said she was utterly stunned by the sudden upheaval engulfing her clinic.

‘I don’t know where to go from here,’ she told AZFamily News at the time. ‘I don’t want to be involved in criminal activity.’

More than 100 doctors, nurses and support staff worked across Fazil’s three businesses, where everything appeared professional and above board from the outside.

Fazil and Hanif were said to have kept an iron fisted grip on the clinic’s billing, deliberately leaving it without an office manager who might have spotted something amiss, a former staffer said.

‘Nobody was aware of what was going on until he got arrested,’ the source explained in June, speaking on condition of anonymity. ‘If it was a con, it was a con that was done very well for a period of time.’

Born, raised and educated in Pakistan, Fazil later became a naturalized US citizen while maintaining close personal ties to his homeland and the glitzy Gulf emirate of Dubai, according to evidence presented at a June bond hearing. 

Prosecutors said his wife owns an operating clinic in Dubai, while Fazil’s brother was previously deported from the US to Pakistan. Bank records allegedly showed roughly $1 million flowing from Fazil’s accounts to Dubai and Pakistan. 

His wife, who helped run their flagship Bio Family Clinic along with two other suspect medical centers in Yuma and a clinic in Dubai, was never formally charged. 

Fazil also reportedly crossed the US Mexico border on foot during his travels and frequently used his private fleet of small aircraft to get around.

According to the Drug Enforcement Administration (DEA), his personal pilot and other close associates have alleged ties to Mexican drug cartels.

Medical staff who once worked for the disgraced doctor previously told Our News Outlet they lost paychecks, scrambled to find new jobs and suffered lasting damage to their professional reputations.

One former staffer said colleagues suddenly lost both wages and access to their 401(k) retirement savings.

‘They got greedy and got busted before he could get out of Dodge,’ a source said. ‘He will probably not see the light of day, and he will probably serve time for it.’
